Fraser welcomes referrals from the community and our referrals system ensures the best care for your clients.

Over 80 years of experience allows Fraser to provide diagnosis and complete assessments resulting in treatment plans tailored to the needs of individuals with special needs and their families. With expertise in autism, mental health, rehabilitation therapy and adult services, Fraser incorporates community resources and approaches all evaluations and treatment plans as a collaborative process.

When you are concerned about the development of your patient, please contact us, using the following as a guideline.

  1. Submit a Professional Referral Form
  2. Fraser Client Services will review and contact the client or family being referred for services to begin the intake process. Families can also call 612-767-7222 to speak with a Client Services Representative Monday-Friday, 8:00 a.m. - 5:00 p.m.
  3. A Fraser Professional Referral Coordinator will review and follow up with you, based on your preferences indicated on the referral form, by tracking your client’s progress through our referral system.

If you want to exchange more detailed information such as records or communication about ongoing treatment, please include a release of information with your referral.
